MASS Clinic Patient Eligibility Criteria

Here are the four criteria an individual must meet to be a patient at the MASS Clinic

Of Age

Patients must be 18 years or older

Patients must be current residents of Duval County

Eligibility: No Health Insurance, Medicare, or Medicaid Coverage

Federal Poverty Guideline

New patient must have an income. at or below 300% of the federal poverty guideline

I was a patient at MASS Clinic for about 7 to 8 months, and my experience was honestly 100% the best. I worked with both a dietician and a physical therapist, and they were extremely experienced, supportive, and helpful with all of my needs. The dietician also enrolled me in Feeding North Florida, which made a big difference for me. I am very grateful to MASS Clinic for accepting me while I was uninsured. I truly love the facility and the entire staff. I only wish I were able to obtain care through MASS Clinic even with insurance, because if the healthcare industry were set up more like MASS Clinic, it would be better for all citizens. SP has been a patient at the MASS Clinic since 2016. Throughout the years, SP has been referred to several specialists, including a neurologist, podiatrist, dietitian, for a mammogram, and a gastroenterologist. SP reports having a great experience with each of these specialists, noting that they listened carefully and addressed every concern with care and professionalism. SP also had the opportunity to speak with a mental health counselor, describing the experience as positive and beneficial. It provided a space for thoughtful conversation and personal reflection. In addition, SP made use of the clinic’s diet counseling services and shared that the experience was extremely helpful. The dietitian introduced new portion control techniques, provided a customized list of suitable foods, and even supplied a visual plate guide to help structure meals more effectively. SP expressed full satisfaction with the services received and did not offer any suggestions for improvement, stating that the MASS Clinic is doing everything well. SP has never had a bad experience and always leaves each visit with answers and a sense of relief, feeling genuinely cared for. SP extended heartfelt thanks to every member and service provider at the clinic for their continued dedication and hard work.

Over the past decade, J.M. has received care from a variety of specialists and programs, including general medical services, an orthopedic specialist, a chronic care management specialist, a gastrointestinal specialist, and the Prescription Assistance Program. She described her experience with the medical team as consistently compassionate and professional. From managing chronic conditions to addressing specific orthopedic and gastrointestinal concerns, J.M. feels that each provider has taken the time to truly listen, understand her needs, and offer personalized, high-quality care. The Prescription Assistance Program has also been a tremendous help, ensuring she has continuous access to the medications she needs without financial strain. Reflecting on her journey, J.M. expressed deep gratitude for the support she’s received over the years. She shared that joining the MASS Clinic has felt like becoming part of a family, one that truly cares. “Today I say thank youuu for your support (service),” she wrote, emphasizing how happy and thankful she is to be part of such a warm and supportive community.

J.C. has been with the MASS Clinic for six months. During this time, he has been referred to several programs that he found both helpful and easy to access. J.C. shared that the Prescription Assistance Program has been particularly beneficial and convenient, and he appreciates the ongoing support from the chronic care management and kidney specialists, noting that they are always available to help. J.C. has also used the clinic's Mental Health Counseling and Psychiatry services. He reported that both the medication and the staff have significantly improved his well-being, helping him feel better about himself. In addition, he has taken advantage of the Food Pantry and Diet Counseling services. He described the food pantry as a valuable resource, especially for fresh fruits and vegetables, and said that the dietary counseling has taught him how to read nutrition labels more effectively, enabling him to make healthier food choices.

A.A. has been a patient at the MASS Clinic for over a year and describes their experience as nothing short of exceptional. Throughout their time at the clinic, A.A. received care from a primary care provider and was referred to several specialists, including a nephrologist, gastroenterologist, cardiologist, and behavioral health specialist. They reported that every provider they encountered delivered care that far exceeded their expectations. Of particular significance was A.A.’s experience with the Mental Health Counseling services, which they described as life-changing—offering a new and transformative perspective on life. While A.A. has not personally used the food pantry, they have witnessed its positive impact on others and recognize it as a vital resource. Overall, A.A. praises the clinic as an outstanding facility providing comprehensive and compassionate care.

“I was referred to MASS Clinic by Barnabas Fernandina about two months ago. My experience here has been excellent. The first encounter right off the bat helped me tremendously and I was referred to follow up for Rheumatology not too long after. Before, I used to be in a lot of pain, but after I was seen here, I was prescribed medication to help ease the inflammation. I’d say my health has improved by, say, 98%.”

“A nurse at Baptist hospital recommended I come to MASS clinic for my foot because I thought something was wrong with it after I got injured. Q. How do I become a patient?

To become a patient, you must meet the following criteria:
Be uninsured. This includes Medicare, Medicaid, or Share of Cost.

Be 18 years or older.

Have an income at or below 300% of the federal poverty guidelines.

Reside in Duval County.*

* If you are a resident in surrounding counties (St. Johns, Clay, Nassau, or Baker), you will be eligible for In-House service. Please contact the clinic for further information regarding In- House Services.

Q. Can anyone become a patient at MASS Clinic?

Anyone who meets our eligibility requirements is welcome. However, as a volunteer clinic, we cannot provide services for pregnant individuals, minors under 18, or offer STD testing, TB testing, or vaccinations.

Q. Are there any costs associated with the services?

MASS Clinic is free of charge for all patients that qualify for our services. Please note that pharmacies will charge for prescribed medications.
Q. Why do we need a proof of income (POI) statement?
Muslim American Social Services
Your POI is required for processing referrals. We cannot arrange specialty appointments without proof of income. The sooner we receive your proof of income, the faster we can proceed with your referral, which generally takes about 6-8 weeks.

We will notify you through the patient portal. If a follow-up is necessary, we will contact you via text.
Q. Do you administer vaccines?

No, but the Florida Department of Health can administer them at a nominal cost.

Sundays from 1:30 PM to 3:30 PM and Tuesdays from 9:30 AM to 11:30 AM. You may visit up to two times a month.
